The Clinico-pathological Study on the Uterine Myoma


Abstract
From February, 1993 to July, 1994, 287 patients who had an abdominal total hysterectomy and confirmed as myoma uteri histologically were reviewed for the clinico-pathological analyses. The study group was classified into the two groups. The
patients
with Hb<10.0 gm/dL were classified into group A, and Hb¡Ã10 gm/dL were classified into group B. Age, parity, symptoms, EKG abnormalities, postoperative febrile morbidity, transfusion, pre- and postoperative Hb level, histological characteristics,
combined disorders were compaired between the two groups.
@ES The results were as follows :
@EN 1. Among the 287 cases 96(33.4%) were group A and 191(66.6%) were group B.
2. No significant diffenences were demonstrated on age, parity, numbers of abortion between the two groups.
3. Menorrhagia or menometrorrhagia was significantly higher in group A(P<.0001), but no significant differences were demonstrated on metrorrhagia vaginal spotting, postmenopausal bleeding between the two groups.
4. No significant differences were demonstrated on lower abdominal pain, pelvic pressure, dysmenorrhea, and back pain between two the groups.
5. No significant differences were demonstrated on palpable mass, leukorrhea and frequency, but dizziness was significantly higher in group A(P<.005), and asymptomatic cases were significantly higher in group B(P<.01).
6. No significant differences were demonstrated on EKG abnormalities, postoperative febrile morbidity between the groups.
7. The number of cases who had transfused in group A were 42(43.8%), and the number of transfusion were 2 - 6 bottles (mean 3.1). The number of cases who had not transfused were 54(56.2%) in group A, and none was transfused in group B.
8. Pre- and postoperative 2nd day Hb levels of the patients who had not transfused in group A were 8.9¡¾0.8 gm/dL, 7.7¡¾1.5 gm/dL, respectively Pre-, posttransfusion and postoperative Hb levels of the patients who had transfused were 6.8¡¾1.4
gm/dL
10.2¡¾2.4 gm/dL, and 9.0¡¾1.1 gm/dL, respectively. Pre- and postoperative 2nd day Hb levels of group B were 12.4¡¾1.3 gm/dL, 11.6¡¾1.2 gm/dL, respectively.
9. No significant differences were demonstrated on the number of cases of submucosal, intramural and mixed type between the two groups, but submucosal type significantly higher in group A(P<.005).
10. No significant differences were demonstrated on weight of the uterus and the largest diameter of the myoma between the two groups.
11. No significant diffenence was demonstrated on the number of the myoma nodules between the two groups.
12. No significant diffenences were demonstrated on secondary degenerations between the two groups.
13. No significant diffenences were demonstrated on combined disorders such as chronic cervicitis, PID endometrial polyp, endometrial hyperplasia, ovarian cyst, endometriosis and carcinoma in situ of the cervix between the two groups, but
adenomyosis
was significantly higher in group A(P<.05).
